Wide GitHub

Doesn't work with enterprise Github installations that have different URLs than the two support ones.

Needs GitHub enterprise support

Does not work on my setup. I have one portrait, one landscape display. It works on the landscape one but is too wide on the portrait one (I have to scroll to see the RHS of comments). Same thing happens if I have a window that is not maximised. Refreshing does not help.

Missing support for custom github URLs. Completely unusable.

I agree with others. This is of limited utility without GitHub Enterprise (GHE) support. 95+% of what I do is on a GHE instance. A simple fix might be to just have the extension work on github.* domains (all of my GHE instances fit this pattern)

No Github Enterprise support? :-(

The comment button is off screen when doing a pull request review to submit the button. Makes reading code much easier, but comments are now more painful and you have to scroll.
